Edit Custom Intake Forms

How to edit custom intake forms. Only one custom intake form can be created for each case type. To edit custom intake forms, the firm must have custom intake activated. Here is how to activate custom intake.

  • NOTE: Only firm administrators or users with the Manage Custom Intakes permission can view this screen.

  • Navigate to the

    Icon in the top right corner of your account and select Custom Intake.

  • Navigate to the custom intake form the firm would like to edit.

  • Click the arrow at the end of its row.

  • Select Edit.

  • A pop-up window will appear.

  • Make the desired changes.

  • Click Save and Close when finished.

  • This will save the changes to the form.

  • The form will be ready to view once you close out of the pop-up window.
