If the health provider's address does not populate in your generated template, be sure to check the following:
Ensure that you have added the correct keywords to your template ( ##{{ provider_office_address_p1 }} and ##{{ provider_office_address_p2 }} )
Ensure that you have entered a complete address for the health provider contact. A street address must be entered in order for the health provider's address to populate into a template. An address cannot be populated if only the City, State, and Zip have been entered.
